While we’d all like to get back to normal, COVID-19 continues to force some IT departments to prioritize support for vaccine programs. In speaking with many clients in the past month, we learned IT staffs are being repurposed to register patients and support a variety of internal efforts. With the vaccine rollout underway, it is clear this effort will impact most healthcare organizations for many months. In light of this, I would like to commend the leadership of MUSE for recognizing this and cancelling the 2021 in-person Inspire conference. Further, MEDITECH has announced its annual Home Health, Nursing and Physician/CIO Forums will be conducted virtually. We support these prudent decisions, despite the serendipity and value of face-to-face networking.

We adapted our call center to serve a variety of purposes and are now taking calls directly from patients, with the ability to schedule vaccine appointments and register patients into MEDITECH and stand-alone scheduling systems (including PrepMod, which is prevalent in many states). In last month’s issue we shared information about a complementary service offered by Navin Haffty to help healthcare organizations identify the tangible, realizable benefits achievable with a migration to MEDITECH Expanse. Using publicly-available Centers for Medicare & Medicaid Services (CMS) data, we customize your current performance on a variety of metrics against industry benchmarks, compared to specific gains other sites have achieved with Expanse. Compiling this report involves little of your time and is not disruptive to you or your organization. Here are excerpts from Navin Haffty benefits identification studies outlining recurring annual improvements identified for or realized by hospitals running Expanse.
Based on a recent Navin Haffty analysis, hospitals on MEDITECH’s Expanse platform outperform those using any other major vendor EHR with regard to clinical quality measures. Expanse tools directly impact avoidance of hospital-acquired conditions, reduce readmissions and have resulted in better performance on value based purchasing measures.

Based on likely improvements in CMS measures, we expect significant gains to be demonstrated. It is anticipated the new tools in Expanse will help reduce readmissions and eliminate the current CMS readmission reduction penalties at (name redacted) Health System, estimated at $597,867 annually, based on the estimated FY2021 CMS readmission reduction penalties at (name redacted) Health System. It is also expected the new tools in Expanse will help improve value based purchasing metrics and eliminate the current negative value based purchasing adjustment at (name redacted) Health System, estimated at $53,282 annually, based on the estimated FY2021 CMS negative value based purchasing adjustment. Navin Haffty expects the new tools in Expanse will help reduce hospital acquired conditions and eliminate the current CMS hospital acquired conditions reduction penalties at (name redacted) Health System, estimated at $65,008 annually, based on the estimated FY2021 CMS hospital acquired conditions reduction penalties. For each of our Expanse clients, there have been significant reductions in readmissions as well as hospital acquired conditions, while achieving improvements in value based purchasing measures.
Through improved Authorization & Referral Management (ARM) tools and tighter linkages between clinical and financial data, Navin Haffty has noted marked reductions in denials for most of our Expanse clients.
Cost Basis (example)
According to the data available, (name redacted) Health System had 46 inpatient Medicare claims denied, a rate of 2.3%, which is above average (per Definitive Healthcare) for short term acute care hospitals with 50-150 beds in (state name redacted) of 1.5%. We believe, at a minimum, Expanse will cut Medicare denials by 50%. This calculation is based on avoiding 23 Medicare denials at the average NPR per discharge. Assuming this is achieved, this translates into an annual reduction in Medicare denials of $1,076,131. Further, we do not have information on the number of denials from other payers, a significant number of which would also be avoided.

Vaccine Support Services
Navin Haffty and Engage are providing vaccination call center scheduling and pre-registration assistance for Lawrence General Hospital. The Lawrence, Massachusetts-based safety net hospital is coordinating a local COVID-19 vaccination clinic in Lawrence for eligible residents of Essex County. The hospital is developing an infrastructure and establishing resources to meet the demands of the volume of vaccination requests it expects, which are projected to increase dramatically from 300 to 2,000+ daily. With limited internal staff to handle the surge, the hospital called upon Navin Haffty/Engage for third-party assistance. In record time, we established a streamlined solution for the site, including establishing a call center for vaccination scheduling, and a process to manage pre-registration.
We are proud to offer this service to Canadian hospitals to support this unique challenge to healthcare.
